Dr. Zhu is the preferred provider for most insurance plans, such as Aetna, Blue Cross Blue Shield, United Healthcare, Cigna, Great-West, PHCS, etc.

If your insurance covers Acupuncture, we will be happy to file the claim for you. We will collect any co-pay/co-insurance at the time of your appointment.

Insurance coverage is not always guaranteed. Each insurance plan has different benefits and we will verify what your insurance covers. • Your full name and date of birth.
• Your insurance name and member ID which is on the front of your insurance card.
• Insurance provider and phone number on the back of your insurance card.
• If the insurance policyholder is someone other than yourself, we will need their name and date of birth.

Medicare or Medicaid plans exclude acupuncture coverage.
